Certificate of Attendance
Certificates of Attendance for all registered delegates are available upon completion of the Evaluation Form.
Please click here for the form.
- Compete the Evaluation Form
- Click on Submit
- Enter your bar code number (The number under the barcode on your delegate badge) on the top right of the page, and then click Go!
- Your certificate with the number of credits available can be printed
